I have automatic updates turned off. I got the nag to update to 18.6 the other day and elected to install it overnight. Then I noticed that it was immediately downloading the update - presumably getting ready to install it at night.
This might be happening for some of the above cases - iOS downloads some (security?) updates “automatically” but holds off installing them?
I have long since done this I just checked and Automatic Updates are still turned off.
Nevertheless, another 600MB of mobile got chewed up yesterday.
Two more things to check, then, are:
Settings > Apps > App Store > Automatic Downloads
and
Settings > Apps > App Store > Cellular Data

Perhaps there are regional restrictions on the page (that would be weird, no?) or proxy/VPN/ISP issues.
