I tried turning off its cell and wifi, and then turn on and off airplane. That doesn’t fix it. I reboot my iPhone 12 mini (updated iOS), and that fixes it. I’m wondering if you guys also run into this very rare issue too.

I’ve had a similar, probably related issue: once every month or two, my iPhone 15 can not make or receive phone calls. Trying to make a call gets “Call failed”, and if called from another phone it never rings. I haven’t checked if the Internet connection is working when this happens. I have Wi-Fi calling with AT&T enabled, so this could be a problem with Internet access as opposed to “phone” access. Turning Airplane mode on and then off sometimes fixes it, but sometimes a reboot is needed.
My wife’s iPhone 11 does this, and it’s really frustrating. It’s been happening about once a week lately. Like @JKBull, the phone has wifi calling turned on, which works great when this problem isn’t happening. The only thing I’ve found that helps is rebooting, though I’ll try toggling Airplane mode next time.
My 3rd generation SE is configured the same way and has never had this problem. We use Spectrum, which I believe is Verizon under the hood.
